Office forms: Before coming in for your first visit, please download and fill out our Patient Registration Form and a Health History Questionnaire.

For your first visit: Arrive ten minutes early to fill out consent forms and get settled. Have a drink of water or make yourself a cup of tea in our waiting room. Allow one and a half hours for your initial consultation and treatment.
For your follow-up appointments: Always eat a little something prior to your appointment. Wear loose, comfortable clothing. We also provide cloth gowns for your comfort. Don't plan on heavy physical exercise for at least two hours after your acupuncture treatment. Try to allow yourself some time to relax after your treatment.
Payment policy: Payment is expected at time of treatment. We will provide you with a Statement which you can submit to your health insurance carrier for reimbursement of your treatment cost if, and to the extent, such costs are reimbursable under the terms of your insurance policy.
Cancellations: We ask you to provide us 24 hours notice of appointment changes or cancellation. This is a courtesy to other patients who may then be able to schedule in your absence. Charges for your missed appointment may otherwise incur.
